Just a guess here, but they could have made a small slit in the fish just big enough to push the capsule through, they would not have had to cut the fish open much. They could have jammed the capsule farther into the fish with a stick. Then they could have sewed the fish back shut with some thread that matched the fishes color (or transparent fishing line.) With that size of fish, it would likely go unnoticed given the stress of the situation.
Second idea that just came to me: Feed the fish the capsule while still alive. Wait a day or so, whack the fish. Now it's not in the stomach where it can easily be pulled out.
